AFTER YOU TALK TO SELF-HELP STAFF TO REQUEST YOUR FORMS OR PACKETS YOU CAN PICK THEM UP FROM THE TABLE IN OUR BASEMENT LOBBY MONDAY THROUGH FRIDAY BETWEEN 8:00 A.M. AND 5:00 P.M.
